Do I need a Referral from another Doctor?
No referral is needed to make an appointment with Dr.
Nichols. Many doctors do refer their patients to Dr. Nichols, but
any patient can make their own appointment.
Appointments
All office visits are by appointment. If you
are unable to keep your appointment, please call our office. We
attempt to schedule appointments in such a manner that delays do not
occur. However, occasions do arise when one of our patients has a
medical problem that requires immediate attention. Due to
scheduling difficulties we are not able to see other members of your
family during your visit, unless you have made previous arrangements
with our scheduling secretary.

Prescription Refills
If you need a prescription refill, please call your
pharmacy first. They will take the necessary information from you
and call our office for approval. Your refill request and your
medical chart will be given to Dr. Nichols to make a decision regarding
the refill. Our office will then call the pharmacy with Dr.
Nichols' decision. Because the doctor must approve all refills,
please have your pharmacy call on Monday or Thursdays.
With this in mind, please do not wait until your
medication is completely out before you call your pharmacy.
Prescriptions are not refilled after normal office hours.
Office Fees
Information about fees for routine office visits and
surgery can be obtained from the office staff. Payment for office
procedures is due at the time of service; this includes co-pays,
co-insurance, and deductibles for your insurance. Credit cards,
checks, or cash are accepted. If you have questions about your
bill, please let us know.
Surgery Fees
Charges for surgery and hospital visits will be
billed to your insurance company for you. Coverage of policies
vary greatly, and you should understand that you are responsible for any
amount not covered by your insurance company.
Medicare
Dr. Nichols is a participating provider. We
bill Medicare and one supplemental insurance for patients with this
coverage. Your deductible must be met first.
Other Insurance
We will bill your insurance for you. Dr.
Nichols is a member of many preferred provider organizations (PPO's).
Co-pays, co-insurance, and deductibles are due for office fees at the
time of service.
Emergencies
All life threatening emergencies are best handled at
the hospital emergency room. In the case of other emergencies, you
should call this office and explain the situation. When the office
is closed call the office number (246-2430) and the answering service
will connect you with Dr. Nichols or the orthopedic surgeon taking his
calls. |
