Preparing for Your Appointment
what to bring, what to expect

Welcome to North Valley Orthopaedics

As your medical needs are met here, you may have some questions about our office procedures.

When you call for an appointment, the receptionist may ask for information about the condition you want to be seen for.  This will facilitate scheduling your appointment.

To enable Dr. Nichols to give you the appropriate care, you will be asked to fill out a medical history form including a list of your medications.

If you have recent x-rays, you may be requested to bring them with you to your appointment.

For billing purposes your insurance cards will be needed and other forms will need your signature.

Your visit with the doctor will begin with an interview, followed by an examination.  Following the examination, x-rays or other diagnostic tests may be ordered.

Your treatment may include medications, casts, medical counseling, exercise therapies, or recommendations for scheduling surgery.  Dr. Nichols will discuss what treatment options are possible and help you select the best treatment plan for you.

You can simplify (and speed up) your visit by completing our Patient Information forms ahead of time and then bring them with at your 1st visit.
